Exeter: A 22-year-old man named Leo Henshaw fell to his death from a sixth-floor flat. This happened just two days after he was unexpectedly released from prison.

Henshaw had a troubled past, including a recent charge for sexual assault. He died on October 7, 2023, after falling from a window while under the influence of methamphetamine.

Before his death, he was known to have mental health issues and a history of substance misuse. He had been in and out of the prison system and had been released from HMP Bristol.

Henshaw had been admitted to a psychiatric hospital in 2022 but was discharged after three months. He faced legal troubles, including assaults, which led to multiple prison sentences.

After his last release, he was supposed to stay at a probation hostel. However, he was charged with sexual assault just days later and returned to prison.

On October 5, he appeared in court via video link and was released on bail. The next day, he missed a probation appointment, raising concerns about his well-being.

Sadly, on October 7, he jumped from a flat window that was not his registered address. His history of mental health issues had been noted, and he was under suicide prevention measures.

Despite efforts from prison staff to support him after his release, the situation ended tragically. An investigation into his death has been completed, highlighting the challenges he faced.
